GUIDELINES FOR DOCUMENTATION
FOR LEARNING DISABILITIES
OR ADD/ADHD

A QUALIFIED PROFESSIONAL MUST CONDUCT THE EVALUATION

  1. LEARNING DISABILITIES
    1. The diagnosis must be made by: clinical or educational psychologists; school psychologists; neurophysiologists; learning disabilities specialists; educational diagnosticians; or medical doctors with training and experience in the assessment of learning problems in adolescents and adults.
  2. ADD/ADHD
    1. The diagnosis must be made by: psychologists of neurophysiologists with comprehensive training and direct experience with ADD/ADHD experience; psychiatrists; and other relevantly trained medical doctors.

DOCUMENTATION MUST BE CURRENT

  1. LEARNING DISABILITIES
    1. For high school students, testing should have been completed within the past three years. For adults, testing should have been completed within the past five years.

  2. ADD/ADHD
    1. A diagnostic evaluation must have been completed within the past three years.

CONTENT

  1. LEARNING DISABILITIES
    1. Comprehensive testing must include measures of both Aptitude/Cognitive Ability and Academic Achievement. Actual test scores must be provided. A specific diagnosis must be included.
  2. ADD/ADHD
    1. A specific DSM-IV diagnosis must be stated.

DOCUMENTATION FORMAT

  1. LEARNING DISABILITIES AND ADD/ADHD
    1. The report must be typed, on letterhead, and contain the signature and credentials of the evaluator.
