VA and Hazelwood Forms



VA Forms for GI BILL Reciepients

(These forms can be sent via email, fax, mail or in person)

Establishing a record forms: All G.I. Bill recipients need the following forms to establish a record in our VA Office.

  1. DCCCD Data sheet
  2. VA Standards of Progress
  3. DCCCD Info Cover Sheet and POC's (optional)
  4. Detailed VA Checklist - For your records.
  5. Tuition Account Summary - With paid amount (aka, paid fee receipts). This is submitted every semester that you desire your Education benefits.

New Students to Richland College using RLC Degree plans (Transfer or New to college) You will need only one of listed forms the box.

Returning Students to RichlandCollege (previously attended at RLC)

State VA Forms for Hazlewood reciepients

These forms can be sent via email, fax, mail or in person.

  1. HE - P - This is required every semester that you desire to use your Hazlewood benefits after initial application.
    1. Prior recipients of Hazlewood - This is required every semester that you desire to use your Hazlewood benefits after initial application.
  2. HE - V
    1. Veterans who have never used Hazlewood
  3. HE - D
    1. Dependents who have never used Hazlewood
  4. Tuition Account Summary or a copy of your CE sign up sheet completed - This is required every semester that you desire to use your Hazlewood benefits.
